Capital Health Plan utilizes Caremark to administer our pharmacy benefits. You can register at the Caremark web-site (www.caremark.com
) to find out more information about your specific medications (including refill history and cost), general information on medications (side effects, storage concerns, and drug interactions), and the benefit of using generics over brand name medications. If you have trouble logging in to Caremark's website, or you do not have access to a computer, you can contact their customer service department at 1-800-966-5772.

Advance Directives and Living Wills
End of life decisions about medical care can be much easier when advance directives are used. An advanced directive simply means a statement, made while you are competent, about the medical treatment you want when you consciously cannot make decisions. Decisions made early and communicated plainly may have tremendous value for the person and the family.
Case Management Program
The CHP Case Management Program is a service offered to all CHP members. Our nurse case managers can offer assistance to members and their families who are facing a serious illness or injury and need help navigating the sometimes complex systems of medical care.
Disease Management
A comprehensive program of disease management, health coaching, decision support, symptom management, and health and wellness education is provided for CHP members who have asthma, diabetes, coronary artery disease, congestive heart failure and chronic lung disease.

Utilization Management
CHP makes health care decisions to achieve quality treatment and efficient use of resources. CHP and its participating physicians coordinate preventive care, evaluate new technologies, and develop clinical treatment tools to guide the care of members. This is called "utilization management."
