Language Assistance

Members in need of language assistance should contact the Member Services department. Member Services Representatives can accommodate non-English Speaking or limited English Speaking (LEP) members and/or prospective members through readily available translation services.

Member Materials

We are pleased to provide you with materials and information to help you learn about our many programs and services and to LIVE WELL as a CHP member. We also offer a New Member checklist to make your first experience with CHP even easier (established members should check it as well to make the most of your CHP benefits.

Request or Re-Order ID Cards

Order ID cards anytime using the ID Card Request link on CHPConnectexternal link icon. If you are not registered for CHPConnect, you can send us an email or call Member Services at 850-383-3311 to order ID Cards.

Select A Primary Care Physician

You can locate a Primary Care Physician using our online Physician and Provider directory. You can notify us of your PCP selection using the Change PCP link on CHPConnectexternal link icon, by sending an email, or by phone at 850-383-3311.

Need Care After Hours?

Contact your Primary Care Provider’s (PCP) office to explain the problem and receive medical direction on treating your condition. If your PCP is not available, Capital Health Plan’s Urgent Care Center is a great alternative. You can contact CHP’s Urgent Care Center by calling 850-383-3382. If you have a medical emergency, get help as quickly as possible. You do not need to get approval or a referral first from your PCP for medical emergencies.

Continuing Your Coverage -- COBRA

If COBRA (Consolidated Omnibus Budget Reconciliation Act) applies to your employer group, you may be entitled to continue your group health coverage for a limited period of time. For more information please contact your group administrator or call Member Services at 850-383-3311.
