Disease Management

Disease management, health coaching, decision support, symptom management, and health and wellness education are provided for CHP members with chronic conditions.

The Health Promotions Department at Capital Health Plan offers comprehensive programs to help members with diabetes and vascular disease manage these conditions. Disease management programs are designed to support and reinforce the treatment plans of each member’s primary care physician. CHP members do not have to enroll; they are automatically enrolled when they are identified as having one of these chronic conditions.

Disease management programs are based on clinical practice guidelines approved by CHP’s Quality Improvement Committee.

In addition to the services provided by Health Promotions, all members are encouraged to call CHP Health Coaches at 850-383-3400 with questions about any health issue, to include care for chronic illness.
